NOTE: On the Credit Card form, type the name of the person attending the conference in both the “Invoice/Ref No/Surname” box as well as in the “Description” box.

1. AWBMAMD Conferences held outside Australia are not subject to Australian GST.
2. If internet credit card payment is successful, the word “APPROVED” will be next to the Response heading. This transaction record will be your Receipt.
3. If internet credit card payment is not successful, other messages will appear next to the Response heading. For example, “CARD INVALID”; “CUSTOMER CONTACT BANK”; etc.
